Our services are Home and Community-Based
Millions of Americans, including children, adults, and seniors, need long-term care services because of disabling conditions and chronic illnesses. Medicaid is the primary payer across the nation for long-term care services. Medicaid allows for the coverage of these services through several vehicles and over a continuum of settings, ranging from institutional care to community-based long-term services and supports (LTSS).
The Centers for Medicare & Medicaid Services (CMS) is working in partnership with states, consumers and advocates, providers and other stakeholders to create a sustainable, person-driven long-term support system in which people with disabilities and chronic conditions have choices, control and access to a full array of quality services that assure optimal outcomes, such as independence, health and quality of life.
The goal of case management is to achieve optimal healthcare outcomes for patients in an efficient and cost-effective manner. By focusing on individual patient needs, case management links patients to health care and community resources and, when necessary, removes barriers to accessing those resources.
Case management partners with children and families to:
· Assist with referrals to specialists/sub-specialists or preventive care services.
· Help obtains assistance for special health-care needs, health education, and financial or other family needs.
· Facilitate intervention for substance use issues, mental health problems, and domestic violence for pregnant women.
· Provide referral to and coordination of prenatal medical care.
· Facilitate early intervention when a concern is identified.
Case management providers can empower patients and families to be active participants in their own health care and to seek solutions to their psycho-social, financial, or educational problems. Case management providers also serve as patient advocates, communicating with community agencies, schools, or other entities on the patient’s behalf.
Case management can benefit all patients. However, certain patients may have a greater need for it. Such patients include:
· Children and adolescents with special health-care needs.
· Children and adolescents with chronic conditions or those at risk.
· Women with high-risk pregnancies.
· Families with limited resources.
For youth with chronic health conditions or disabilities who are approaching adulthood, case management services can help access transition services. Those transition services address issues that may arise when children become adults, such as the change from pediatric health-care services to adult health services, independent living arrangements, plans for secondary or post-secondary education, attendant care, social support, employment, vocational goals, and legal matters
