CHILDREN AND PREGNANT WOMEN
What is Case Management?
Case Management for Children and Pregnant Women is a Texas Medicaid benefit for eligible patients who have medical-related needs that might affect their health care. The patients must be eligible for Medicaid and be either:
- A child birth through age 20 with a health condition or health risk.
- A woman of any age who has a high-risk pregnancy.
Many of these patients have complex needs that range from specialty referrals and medical equipment/supplies to educational, psychosocial, financial, and transportation services. Case managers, who are licensed social workers or RNs, help patients and their families find and get these services.

Psychological Evaluations
Psychological evaluations are performed by our Nurse Practitioner via TeleHealth.
In addition to providing services to individuals and families, we have partnered with:

- Disruptive behavior in class
- Violent behaviors and threats to cause harm
- Frequent emotional outbursts

- Students with learning difficulties/disabilities
Mental/Behavioral Health Case Management
Community mental health case management services is provided by qualified professionals to assist clients with their recovery goals in the home and in the community
They are responsible for:
- Performing a comprehensive needs assessment
- Locating resources based on needs
- Assist client obtain services
- Creating an individual service recovery plan
- Assisting with day-to-day coping challenges while encouraging independence in the community
- Psychosocial rehabilitation
- Medication management
- Skills Training
- School Advocacy
- Social and Emotional Learning
SERVICES OFFERED
TARGETED CASE MANAGEMENT
AND
MENTAL HEALTH REHABILITATION
Targeted Case Management services are case management services to clients within targeted groups.
The target population that may receive Mental Health Targeted Case Management (MHTCM) as part of the Texas Medicaid Program are clients, regardless of age: with a
- single diagnosis of chronic mental illness
- or a combination of chronic mental illnesses
- and who have been determined via a uniform assessment process to require MHTCM services.
- MHTCM services are furnished to assist clients in gaining access to needed medical, social/behavioral, educational, and other services and supports.
MHTCM activities and services include:
A comprehensive assessment and periodic reassessment, as medically necessary, of individual needs to determine the need for any medical, educational, social/behavioral, or other services.
- The development (and periodic revision, as medically necessary) of a specific care plan that:
- Is based on the information collected through the assessment;
- Specifies the goals and actions to address the medical, social/behavioral, educational, and other services and supports needed by the client;
- Includes activities such as ensuring the active participation of the eligible client and working with the client (or the client’s authorized health care decision-maker) and others to develop these goals; and
- Identifies a course of action to respond to the assessed needs of the eligible client.

- Making referrals and performing other related activities, such as scheduling an appointment on behalf of the client, to help an eligible client obtain needed services and supports, including activities that help link a client with
- Medical, social/behavioral, and educational providers; and
- Other programs and services that are capable of providing needed services to address identified needs and achieve goals in the care plan.
- Monitoring and performing the necessary follow-up that is necessary to ensure the care plan is implemented and adequately addresses the client’s needs.
